Job Requirements

The Care Manager Associate (CMA) has primary responsibility for proactively managing and coordinating health and medically-appropriate services for members who have mild medical and mental/behavioral health needs.  This care will be closely and continually coordinated with the primary care physicians and care givers, and the member’s living breathing care plans will be highly individualized to incorporate their medical, social, psychological, and behavioral needs and challenges.

  • Get to know your members (and their care givers) through comprehensive intake assessments.  Encourage ownership and confidence in their ability to manage their health.
  • Develop and manage patient-centered and culturally competent care coordination plans, ensuring complete, current and compliant documentation of member’s health care needs.
  • Meet regularly with your members to empower and drive positive progress with regard to established milestones of health.
  • Support members with appointment-setting and referrals to preferred provider networks for medical, therapy, behavioral health and home care services.
  • Analyze reports and assessments from providers (including education) to determine appropriate course of action and facilitate coordination of necessary services.
  • Partner with primary care providers (PCP) to ensure current health data is communicated with regard to the “total” member - medical condition, social determinants, and compliance with Plan guidelines that affect the member’s ability to adhere to mandatory established health markers and quality indicators.
  • Serve as a resource to members and their families, providing supportive counseling and completing/following-up on referrals.  Educate providers, the health care team, and the community about case management services through CoordinationPlus.

  • You have 2+ years of related experience in managed care and/or case management.  You’ve maximized your role within your community, guiding and directing your fellow citizens to services that have help to meet their needs.
  • You meet the minimum education requirement of a High School Diploma.  A Bachelor’s degree is preferred.
  • Your clients love you.  Your team relies and depends on you.  You are a valuable, indispensable professional who recognizes the critical nature of your role.  You provide the highest level of service to all those around you.
  • You are technology savvy.  You understand that data is key and you get excited about fully utilizing the systems and applications available to you.
  • You’re an excellent communicator.  You connect personally with everyone you meet.  You are quite adept at entering into engaging conversations for the purpose of gathering critical health information from your clients as well as sharing clinical data with providers.  Not to mention you have highly developed writing abilities as well.
  • You are proactive, creative and innovative.  You know your members and what it takes to motivate them.  You take initiative and are always one step ahead.  You develop creative ways to get results.
  • You are highly organized.  No matter where you work that day your files are always at your fingertips, ready to be referenced at a moment’s notice.  Your organizational abilities are in part what makes you so successful.
  • You respect and work within established requirements.  You have the ability to interpret and apply departmental requirements and are able to use good judgment to make sound decisions.
  • You meet your deadlines.  You have the ability to work independently and carry out assignments to completion.

The Care Manager Associate is not your regular 9-5 job.  Some evening and weekend work may be required based on the schedules of providers and members.  The Care Manager Associate is also a field-based role.  The position will spend the majority of the time in the provider offices as well as community based sites.  The sites may change based on member and staffing needs and requirements.  As such, the CMA must have a valid driver’s license and have or have access to a reliable motor vehicle.
