The HSC Health Care System, in partnership with the DC Health Matters Collaborative, is proud to release our 2019 Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP)!
The DC Health Matters Collaborative is a coalition of hospitals and federally qualified health centers (FQHCs) that combine efforts and resources to assess and address community needs. The work is done in partnership, is data-driven, and engages the community. The ultimate pursuit is an equal and sustainable state of health for District of Columbia residents. HSC is proud to participate in this work and we encourage you to click below and view the reports.
DC Health Matters provides a one-stop resource for online access to community health indicators and related resources that impact the health of DC communities. You will find up-to-date demographic, health, and social determinants data; hundreds of maps, tables, and figures; and promising practices on this website.
Thank you to the hundreds of DC community stakeholders for your thoughtful input and engagement in the 2019 CHNA. The CHNA serves as the community-driven foundation for the CHIP, our shared roadmap for action. HSC, in partnership with the Collaborative, is working on system changes and policy actions to drive towards greater health equity in DC.
